ورجّح خبير في الحيوانات البرية أن يكون وزن أنثى الدب السوداء نحو 75 كيلوغراما، موضحاً أن صغيريها يبلغان بضعة أشهر فقط.

– الدكتور البرغوثي: إزعاجات وأعراض ومطلوب من المحيطين التفهم

رام الله- “القدس” دوت كوم- روان الأسعد- في عمر المرأة مراحل كثيرة تمر بها، لعل أبرزها سن اليأس “سن الأمل”، وإن صح التعبير “فترة ما بعد الخصوبة”، باعتبارها مرحلة جديدة، متطلباتها مختلفة، قد نجد فيها نساء لم يفارقهن النشاط، وبدأن ولادة حقيقية جديدة، وأُخريات قد يعانين من أزمة نفسية وروحية وعدم رضا ومخاوف لا تنتهي، فالأمر مرهون بالدرجة الأولى بالمرأة نفسها، لذلك على المرأة تقبّل الأمر، فهو ليس بداية النهاية، بل بداية مرحلة عمرية تمتلك فيها المرأة جسدها وروحها من دون أن تفقد شبابها، الخيار متروك للمرأة ذاتها، فإما حياة جديدة متجددة مقياسها الأمل وحصاد السنين، أو رمادية لم يبقَ منها إلا مجرد ذكرى بشعة وأوقات صعبة.
ما هو سن الياس؟ وما علاماته ومظاهره؟ وكيف يمكن للمرأة التعاطي معه بالشكل الصحيح وتحويله لسن الأمل والتفاؤل والنضج والتمسك بالعطاء؟ كل هذه الأسئلة طرحتها “القدس” على الدكتور عدوان البرغوثي، الاختصاصي في الأمراض والجراحة النسائية والتوليد والعقم وأطفال الأنابيب ورئيس جمعية الخصوبة الفلسطينية والمدير العام لمركز الهبة لأطفال الأنابيب.
تحدث الدكتور البرغوثي بداية عن مرحلة سن اليأس، مشيراً إلى أن تسمية سن اليأس التي فُرضت على المرأة غير متوافقة مع العصر الحالي، وتسميتها الصحيحة والمناسبة هي “فترة ما بعد الخصوبة”، ليُعرفها بأنها فترة من فترات العمر تبدأ بالمعدل بعد العمر (٤٨-٥٢ عاماً) تتلخص بانقطاع فترة الطمث نتيجةً لنفاد مخزون البويضات عند المرأة، التي تكون مصحوبةً ببعض الإزعاجات والأعراض التي أولها انقطاع فترة الطمث أكثر من ستة أشهر، إضافةً إلى الشعور بالهبات الساخنة والتعرق الليلي مع احمرار الوجه وارتفاع في هرمون FSH (Follicle-stimulating hormone)، وهو الهرمون المنبه للجريب أو الهرمون المنشط للحوصلة الذي يعمل على المبيضين ليحفز نمو البويضات عند الأنثى، إضافة إلى بعض المشاكل أثناء ممارسة الحياة الزوجية والناتجة عن الجفاف وقلة الإفرازات المهبلية التي تؤدي إلى التهابات المسالك البولية والمهبلية، إلى جانب نقص الرغبة الجنسية.
ويضيف: كما يصاحبها أيضاً القلق والتوتر والوضع النفسي السلبي الذي ينعكس على نفسية المرأة بسبب المفاهيم الاجتماعية المغلوطة التي تعتبر المراة آلة للإنجاب، وبيولوجياً فترة ما بعد الخصوبة تعني انقطاع الطمث، فهي كأي مرحلة عمرية تمر بها الأنثى، إلا أن المجتمع بأفكاره السلبية يزيد الضغط النفسي للمرأة كأنها آلة وتعطلت، لأنها فقدت قدرتها على الإنجاب، ما يزيد شعورها بأنها فقدت إحدى أهم الوظائف التي تقوم بها، ما يعني أن هذه الفترة مؤشر إلى أنها دخلت في مرحلة من مراحل العمر المتأخرة، وهو ما يزيد من حالة التوتر والقلق.

وعي المحيطين بالمرأة
ويتابع: وهنا من المهم جداً وعي المحيطين بالمرأة بحساسية هذه الفترة وخصوصيتها وكيفية التعامل معها، خاصة أن هذه الفترة تصاحبها تغيرات هرمونية (ارتفاع هرمون FSH، وانخفاض الهرمون الأنثوي الأستروجين)، وبالتالي تكون المحصلة لهذه التغيرات الهرمونية وانقطاع الطمث دوراً كبيراً في التسبب بالحالة النفسية السيئة وزيادة التوتر والقلق.
ويشير البرغوثي إلى أن هذه الأعراض لا تمر بها كل النساء، فبعضهن تكون الفتره الانتقالية لديهن ما بين انتظام الطمث إلى انقطاعه فترة سلسلة وسهلة، والبعض الآخر تترك لديهن أعراضاً شديدة ومزعجة جداً تحتاج الأنثى خلالها للعلاجات التي تتضمن نصائح، أهمها ممارسة الرياضة مع الحفاظ على نمط التغذية المتوازن والمحتوي على كمية كافية من الكالسيوم لتتجنب فيما بعد هشاشة العظام، كما أنه من الممكن في هذه الفترة إعطاء الداعم الغذائي على شكل حبوب الكالسيوم، لأن نقص الهرمون الأنثوي (estrogen) يؤدي إلى نقص في امتصاص الكالسيوم في الجسم، وبالتالي يتطور إلى هشاشة عظام.
ويستطرد: لذلك أُكرر بأن التغذية المتوازنة مهمة، وحتى عند اتباع برامج الحمية للتقليل من الوزن يجب أن تكون متوازنة تحتوي على كميات كافية من البروتينات والكربوهيدرات والفيتامينات المتنوعه والكالسيوم، كما أن اللباس الذي ترتديه في هذه الفترة يجب أن يكون من الأقمشة الخفيفة التي تمنح شعوراً بالبرودة، وأن يكون قطنياً، كونها تشعر بالهبات الساخنة، ليخفف من الشعور بالحرارة ولا يزيده.

اللجوء لعلاجات غير هرمونية
ويلفت البرغوثي إلى أنه “من الممكن أيضاً أن نلجأ إلى بعض العلاجات غير الهرمونية، كالحبوب التي تحوي هرمون الإستروجين من المصادر النباتية كالصويا، وإعطاء بعض الأدوية التي تحتوي على أنواع من الفيتامينات كي تساعد على تخفيف هذه الأعراض، وفي الحالات التي لا نحصل فيها على النتيجة المطلوبة من الممكن إعطاء الهرمون البديل، وهو أدوية هرمونية بديلة “ثنائية الهرمون” شبيهه بحبوب منع الحمل، تحتوي على هرومون الأستروجين وكذلك البروجستيرون، وتعمل على تعويض النقص لهرمون الإستروجين الذي يسبب نقصه هذه الأعراض”.
ويتابع الدكتور البرغوثي: هذه الأدوية من الممكن أن تعمل على تأخير فترة مابعد الخصوبة، كونه ينتج عنها طمث مصطنع، لأنها بالأساس تعمل على تعويض نقص الهرمونات، إضافة إلى أن الطمث المصطنع يلعب دوراً في تحسن الحالة النفسية للأُنثى والتخفيف من أعراضها، إلا أن هناك محاذير لاستخدام الهرمونات، فلا يتم اللجوء إليها إلا في حالة عدم السيطرة على الأعراض بالأدوية غير الهرمونية، لما لها من أعراض ومضاعفات جانبية، من ضمنها زيادة نسبة الإصابة بسرطان الثدي، لذلك لا يتم اللجوء إليها إلا بعد التأكد من خلو التاريخ العائلي للمرأة، وإجراء فحص الثدي، والتأكد من عدم وجود أي شكوك لوجود كتل أو أورام، وبعد استشارة الطبيب للتأكد من أنها ليست عرضة للإصابة بهذا السرطان، أو لديها أي احتمالية للإصابة به.

العمر.. وعاملان رئيسيان
ويضيف: أما بالنسبة للعمر الذي تتعرض له الأنثى لهذه المرحلة، فيعتمد على عاملين رئيسيين، أولهما العامل الوراثي، لأن له علاقةً بمخزون البويضات، فكلما ارتفع المخزون أكثر تأخرت فترة انقطاع الطمث، والعكس صحيح، ففي كل شهر تفقد المرأة بويضة وينفد المخزون في وقت مبكر، كما أن هذا المخزون يتأثر بالوقت الذي بلغت فيه الأُنثى، فكلما كان البلوغ في سن مبكرة (في سن 9 أو 10 أعوام)، كانت فترة انقطاعها مبكرة أكثر لانتهاء مخزون البويضات في سن أصغر، أما العامل الثاني فهو عدد الأطفال الذين أنجبتهم المرأة، والعلاقة هنا طردية، فكلما ازداد عدد مرات الحمل والولادة، تأخرت فترة انقطاع الطمث، لأن فترة الحمل والإرضاع تمنح المرأة وقتاً ينقطع فيه الحيض، وهذا يؤدي إلى توفير عدد كبير من البويضات، وبالتالي التأخر في الوصول إلى فترة ما بعد الخصوبة، لذلك يجب على النساء بشكل عام اللجوء للاستشارة الطبية إذا توقف الطمث عندهن أكثر من شهرين من أجل إجراء الفحوصات، والتأكد من أن أسباب الانقطاع للدورة الشهرية ليس مرضياً نتيجة لوجود أكياس على المبايض على سبيل المثال، وليس على سبيل الحصر.
ويشير الدكتور عدوان إلى أن الأصعب هو انقطاع الطمث المبكر، الذي يحدث عند البعض في مراحل العمرية مبكرة كالعشرينيات والثلاثينيات، كونه من الممكن أن يحرم الأنثى من الأمومة، وفي حال حدوثه للأسف الشديد لا توجد له علاج في المستقبل وتصاحبه الأعراض نفسها، لكن الحالة النفسية هي الأصعب والأشد، وبالتالي من المهم جداً إذا كان انقطاع الدورة الشهرية في عمر مبكر قبل بلوغ الـ45 عاماً يتم اللجوء إلى الحبوب الهرمونية التي تعمل على إنزال الطمث حتى وصولها إلى العمر الطبيعي لانقطاع الطمث، بعد اتباع كل الإجراءات الطبية والفحوصات اللازمة.
ووجه الدكتور عدوان البرغوثي رسالة للنساء في هذه المرحلة العمرية من أجل تحويله إلى سن الأمل والعطاء بالحديث مع الطبيب والاستشارة الطبية، لأخذ النصائح والإرشادات، ومن الأهمية بمكان إدراكهن بأن هذه المرحلة ما هي إلا مرحلة عمرية ولها خصائصها وليست مرضية.
وشدد على أهمية تنمية المرأة لذاتها من ناحية العمل والوظيفة، كونها في كامل عطائها وأوج قدرتها على الاهتمام بذاتها، ونصح بممارسة الرياضة اليومية وتناول الفيتامينات المتعددة والكالسيوم، لأن نقص الهرمون الأُنثوي يؤدي عملياً إلى نقص في امتصاص الكالسيوم في الجسم، وبالتالي يتطور إلى هشاشة العظام، وأن تكون تغذيتها متوازنة، والأهم أن يكون المحيط بالمرأة متفهماً تماماً لهذه التقلبات التي تمر بها واستيعابها بشكل صحيح والوقوف معها.

وتعد السرعة السبب الرئيسي لحوادث المرور في الدولة الواقعة في شمال إفريقيا، بحسب وكالة حكومية معنية بالسلامة المرورية.

وبعد تشكيل حكومة اليمين الاسرائيلي المتطرف والعنصري، قامت وتقوم هذه الحكومة باستهداف الحركة الأسيرة الى جانب كل أبناء شعبنا في فلسطين، في محاولة للنيل من صمودهم وكرامتهم من خلال سلسلة تقليصات وسلب العديد من المكتسبات التي حققوها عبر تضحيات جسام.
غير ان الحركة الأسيرة مصممة على مواجهة هذا الاستهداف وهذه الهجمة الشرسة، مثلها مثل بقية أبناء شعبنا من خلال مختلف الطرق النضالية وعلى رأسها الاضراب الشامل عن الطعام كسلاح أخير يتم اشهاره أمام سلطات السجون ومن خلفها دولة الاحتلال وحكومة بنيامين نتنياهو الاكثر يمينية وعنصرية.
واعلان الحركة الأسيرة أمس عن سلسلة خطوات لمواجهة هذه الهجمة والاستهداف والتي تبدأ بالعصيان وتنتهي بالاضراب الشامل عن الطعام في الاول من شهر رمضان المبارك، يدق جرس الانذار أمام دولة الاحتلال بأن الحركة الأسيرة لا يمكنها أن تستسلم أو ترضخ لهذه الهجمة الشرسة التي سيكون مصيرها الفشل، كما كل الاستهدافات السابقة.
كما ان هذا الاعلان من قبل الحركة الأسيرة يستدعي من الكل الفلسطيني اعداد العدة للوقوف الى جانب أسرى الحرية الذين ضحوا بحريتهم من أجل حرية شعبهم ووطنهم الذي لا يزال يئن تحت وطأة الاحتلال بانتظار يوم الخلاص.
فليس من المعقول ان فعاليات التضامن مع الحركة الأسيرة تقتصر فقط على أهالي الأسرى وبعض المتضامنين الآخرين، ان من الواجب الوطني والديني والاخلاقي ان تكون فعاليات التضامن شاملة للكل الفلسطيني وان تضم خيم الاعتصامات كافة قطاعات شعبنا.
وعلى المنظمات المعنية بشؤون الاسرى وحقوق الانسان مضاعفة جهودها ونشاطاتها على مختلف الاصعدة والمستويات المحلية والعربية والاسلامية والعالمية، لضمان أكبر وأوسع قاعدة تضامن مع الحركة الاسيرة التي تستعد لخطوات نضالية ضد التغول الاسرائيلي ضدها.
وعلى السلطة الفلسطينية ممثلة بمختلف الوزارات مضاعفة تحركاتها والتوجه من جديد لمختلف المحاكم الدولية ورفع قضايا ضد دولة الاحتلال على استهدافها للحركة الأسيرة ومحاولات النيل منها.

Repeating the phrase "two-state solution" has become an empty slogan

For decades, the solution to the Middle East conflict has centered around one diplomatic phrase: the "two-state solution." Originally, it was clear that the slogan symbolized a concrete idea - the establishment of an independent Palestinian state alongside Israel. But for most politicians it has become an empty slogan, repeated unconvincingly and without any real desire to see it implemented.


Watch, for example, what was announced about the call of US Secretary of State Anthony Blinken with Palestinian President Mahmoud Abbas. According to the US State Department statement, "Blinken stressed that the United States is committed to improving the quality of life for the Palestinian people in tangible ways, and that the (Biden) administration supports a two-state solution through negotiation.


There is nothing new in this talk. Although the United States officially recognized in Israel its acceptance of the 1947 partition resolution, it still does not recognize the Palestinian state mentioned in the same partition resolution. Since that time, Israel has occupied the rest of the areas designated for the Palestinian state, and although the United States and most of the rest of the world have called on Israel to end its occupation of millions of Palestinians, it has not done anything to change the status quo. And this is despite the fact that the United Nations has recognized Palestine as a non-member state, and 139 countries in the United Nations have officially recognized the State of Palestine, but the United States, the European Union, Japan, and Australia did not take the same important decision.


Yet American officials continue to routinely repeat the phrase "two-state solution," which has become a meaningless phrase. And since few expect the United States to boycott Israel or arm the Palestinians (as it works with the Ukrainians), there are practical steps that the United States and the international community can take so that the phrase two-state solution is translated and so that it is not an empty slogan.


For example, the United States can withdraw the name of the Palestine Liberation Organization - which signed the Oslo Accords with Israel - from the list of terrorist organizations. The United States could also ban the import of any items manufactured in illegal Jewish settlements that bear the label "Made in Israel," an incorrect source statement. It could also demand that Israel stop its daily use of violence against Palestinians and work for justice for Sherine Abu Aqleh, an American-Palestinian journalist who American officials have acknowledged was likely killed by an Israeli bullet.


America can do more to help the Palestinian people, building the institutions of state that are necessary to end Israel's control of the land, water and air in Palestine. Israel limits the use of water for Palestinians, puts in place measures to prevent Palestinian construction in the occupied territories, permanently demolishes homes, and refuses to even update the transmitters of Palestinian cellular phones, while allowing the construction of illegal Israeli towers in the occupied Palestinian territories.


Under these circumstances, it is impossible for an independent Palestinian economy to be established, but support for improvement and better economic interaction with neighboring Arab countries, especially Jordan and Egypt, will have an important role in ending compulsory dependence on Israel.


There should be a Palestinian police force at the Karama crossing, the only crossing between the West Bank and Jordan, and Palestinians should be allowed free movement between the West Bank and the Gaza Strip. Israel itself complied with these two demands when it signed the Oslo Accords.


Political developments indicate that all this is possible. When US President Biden visited the region, Israeli centrist President Yair Lapid was prime minister in a caretaker government. Unlike former right-wing Prime Minister Naftali Bennett, Van Lapid supports the two-state solution and is a supporter of negotiations with the Palestinian leadership.


Lapid's term may be short, especially since the Israelis are heading to general elections to choose a government next November. Once again, the Palestinians have to wait for the results of Israel's elections, which has become a waste of time.


That the land between the Mediterranean Sea and the Jordan River should be divided into two independent states or that the two peoples be shared in a state in which all citizens are entitled to equal rights.


Israeli and American leaders who believe in a two-state solution must prove that they mean what they say. At the very least, America should demand - under the threat of sanctions - that Israel cease any actions that might affect the establishment of a contiguous Palestinian state.


When Biden was vice president, the Obama administration authorized Security Council Resolution 2334, which declared Israeli settlements in the Palestinian territories occupied in 1967 illegal and demanded that Israel prevent any further settlement construction in the occupied territories. However, after Biden leaves, Israel is scheduled to announce approval of a settlement plan that will prevent the connection between the north and south of the West Bank.


The continuation of Israeli settlements turns the idea of two states and the leaders who support it into a laughing stock. The question remains: Is Biden serious when he talks about a democratic Palestinian state alongside the State of Israel? In the absence of a real change in policy, the "two-state solution" slogan will be an empty and meaningless slogan.
